Najpierw TK:CT Chest/Abdo/Pelvis/ With Contrast :
Findings :
1. There is no significant mediastinal or hilar
lymphadenopathy.
2. There is an 8mm subpleural nodule in the right lower lobe (image 93) and a further tiny sub-fissural nodule on image 71. There is a tiny left upper lobe nodule on image 52, a tiny subpleural nodule in the left lower lobe on image 102 and minor left fissural nodularity (image 66).
3. There is no significant abdominal or pelvic
lymphadenopathy.
4. There is a thoracic scoliosis concave to the left.
CONCLUSION :
Multiple pulmonary nodules. All are subpleural and seem more likely to represent subpleural nodes than metastases in the absence of any abdominopelvic lymphadenopathy. However, further CT follow up is recommended.

Macroscopic ReportThe specimen is labelled with the patient's name and details.
A radical orchidectomy specimen is received overall 120 x 60 x 20 mm. The spermatic cord and connective tissue are 90 mm in length x 12 mm in diameter. The testis is 60 x 30 x 20 mm. On bivalving the testis there is a circumscribed solid and focally cystic tumour located towards the lower pole. This is 15 x 13 x12 mm in cross surface. The tumour is mostly solid, light pink. Where the tumour is focally cystic the cysts do not contain any substance, there is no area of calcification, haemorrhage or necrosis seen. The tumour is at least 5 mm from the rete testis, the tumour does not abut the tunica albuginea. On serially sectioning the testis a further smooth walled cystic space is identified. This is at least 4 mm from the main tumour. The cyst is 5 mm in diameter. There are no other lesions identified and the background testis aside from the lesions is
unremarkable. Sections from this specimen are submitted as follows:- A,B and C cord; A = resection margin, B = mid cord B, C = proximal cord. D = large block showing tumour, background testis,
rete, epididymis. E through to G = further representative sections of tumour. H and I = the separate cyst identified. K = apparently normal background testis.
Microscopic ReportThe tumour identified macroscopically in the lower pole of the testis is a mixed germ cell tumour, with focal necrosis. The tumour is predominantly yolk sac (80%) with teratoma (15%), very focal
embryonal carcinoma (1%, block F), and also multiple very small foci of intertubular seminoma (<5%) which is noted within the stroma of the rete testis. There is also pagetoid spread of the seminoma within the rete. The immunohistochemical profile of the tumour is supportive of the presence of
these components (yolk sac tumour; Glypican 3 positive and focally AFP positive, embryonal carcinoma; Oct3/4 positive, PLAP, glypican 3, AFP negative (CD30 is very weakly positive in this focus), seminoma; Oct3/4 and PLAP positive). The cyst noted within the testis apparently separate to the main tumour is lined by simple epithelium (pan CK positive) and contains a small amount of smooth muscle within its wall. As such, this has been interpreted as a probable separate focus of
teratoma. Although the tumour size was measured as 15 mm in maximum dimension macroscopically, given the presence of intertubular seminoma, this may be an underestimate of true tumour size. The tumour
is confined to the testis. There is intratubular germ cell neoplasia and atrophy in the background testis, although residual spermatogenesis seen. There is an apparently artefactually misplaced seminiferous tubule within the spermatic cord (block G) which shows the presence of intertubular germ cell neoplasia. There is no lymphovascular invasion.
Also noted is the presence of a single focus of necrotising vasculitis within one of the medium sized arteries within the epididymis (block D). Whilst isolated arteritis of the epididymis is recognised, it is uncommon, and it is a diagnosis of exclusion which can only be made once a systemic vasculitis (such as polyarteritis nodose) has been excluded. In view of this, close clinicopathological correlation is advised, and this will be discussed at the germ cell MDT.
Dr Verrill has kindly reviewed this case and concurs with this interpretation.
SUMMARY
RIGHT TESTIS - MIXED GERM CELL TUMOUR (80% YOLK SAC, 15% TERATOMA, <5% INTERTUBULAR SEMINOMA, 1%
EMBRYONAL CARCINOMA), pT1 Nx Mx, WITH FOCAL ARTERITIS, PLEASE SEE REPORT
Pozdrawiam
